In a Nutshell

You have options between Core and Plus dental plans on Guardian or Collective Health for Guardian. The Core plan preserves a $0 premium option (employee only) but adds an in- and out-of-network deductible ($50 individual / $150 family) and lowers orthodontia coverage. The Plus plan has no deductible and higher basic, major services and orthodontia coverage.

How It Works

You choose your dentist, and you're covered on your first visit. Whether you go the Collective Health platform route or the Guardian platform route, you get the best cost savings when you see a provider in the Guardian PPO network.

Opting in to the Collective Health-managed plan means you'll be sharing your dental info with a group of your coworkers. But don't worry, they're a trustworthy bunch. The details of your dental care will always be kept 100% confidential.

Benefits to You

  • Coverage begins on day one.

  • We cover 100% of premiums for employees on Core plans, and 80% of premiums for dependents.

  • If you need to see a dentist before your enrollment is complete, don't worry. Coverage is retroactive to your start date and can be reimbursed.

  • Guardian pays up to $2,000 per year per person for covered services. Keep in mind that you'll have to pay out of pocket once you reach your annual maximum of $2,000.

How To Enroll

Flock is our online benefits enrollment system. Once you begin work at Collective Health, you will be able to log in to Flock via Okta to confirm your demographic information and submit your benefit elections. If you have dependents that you want to enroll, you'll need to add them as a dependent on your profile before making your benefits selections (be sure to select them on each benefit enrollment page that you would like them added to)!
