The Details

Medical Plans

You still have the option between Core and Plus Medical plans with Blue Shield of California (administered on the Collective Health Platform).

We are excited to share that employee premiums are not changing for 2027! To make that possible, and to address the rising healthcare costs across the industry, starting January 1, 2027, the following plan design changes will be adopted on the Core and Plus plans.

For more information on our Medical plan options and coverage, please navigate to the Collective Health Welcome Portal below!

Dental Plans

There are no changes to our Dental plans. You still have the option between Core and Plus Dental plans on the Collective Health Platform:

  • The Core plan has an in- and out-of-network deductible ($50 individual / $150 family), higher coinsurance and lower orthodontia coverage.

    • This is a great option for those who typically only use preventive care (e.g. oral exam, cleanings, routine x-rays) and want to pay lower premiums (employee premiums are 100% covered).

  • The Plus plan has no deductible and higher coverage but comes with cost share for employees and higher overall premiums.

More information on our Dental plan options and coverage can be found on the Collective Health Welcome Portal!

Vision Plan

No changes to our Vision plan - employee premiums are still 100% covered. You still have the option for either a $250 frames allowance ($135 if purchased from Costco), OR a $400 allowance for contact lenses in lieu of new frames.

More information on our Vision plan coverage can be found on the Collective Health Welcome Portal!

NOOM

Starting on January 1, 2027, all members enrolled in a Collective Health Medical plan will be able to utilize NOOM! Through NOOM, you will have the ability to access the NOOM Health Metabolic Suite, including:

  • NOOM Weight - a scalable, psychology based weight management solution

  • NOOM DPP (Diabetes Prevention Program) - a CDC Full Plus recognized DPP designed to proactively reduce the risk of Type 2 Diabetes.

  • NOOM Diabetes - a behavior change program empowering users with Type 2 Diabetes to manage blood sugar, weight, and overall health.

  • NOOM Med - comprehensive evidence based clinical care across both preventive health and condition management solutions.

Along with the implementation of NOOM, all weight-loss GLP-1s will now be administered through the NOOM Med program and no longer through our Pharmacy Benefit plan. More information about the NOOM program, including FAQs, can be found here!

HRA — via Forma

No change to the Health Reimbursement Arrangement (HRA), which helps to offset Medical, Dental and Vision costs when you opt into any of the Core Medical plans.

  • Enroll in any of the Blue Shield Core Medical plans and the company will contribute $375 for Employee Only plans or $750 for all other enrollment plans (Employee + Spouse, Employee + Family, etc.)

  • HRA dollars can be used to fund co-pays, out-of-pocket expenses towards your deductible, prescription costs, among a variety of other eligible expenses.

More information about the HRA can be found here.

FSA — via Forma

If you are enrolled in a Healthcare FSA for the 2026 plan year, up to $680 of your unused balance can rollover to the 2027 plan year. Dependent Care FSA does not have rollover but instead has a 2.5 month grace period which allows claim expenses to incur until 3/15/2027 for the 2026 plan year’s contribution (must be submitted by 3/31/2027).

The 2027 Healthcare FSA will only apply to eligible expenses between January 1, 2027 - December 31, 2027. If you leave the company, your FSA will end on your final day.

NOTE: The IRS has not released the 2027 FSA Limits. The limits shown below are the 2026 FSA limits.

  • 2027 Healthcare FSA - up to $3,400 pre-tax dollars can be set aside for healthcare eligible expenses like medical, dental, vision costs, prescriptions, copays and/or out-of-network expenses. More details for Healthcare FSA here.

    • Rollover up to $680 unused dollars to the following plan year (2028).

    • Reminder: Over-the-counter (OTC) medications (e.g. Tylenol, Zyrtec, cold medicine) and feminine care products (e.g. pads, tampons) are now FSA eligible.

  • 2027 Dependent Care FSA (DCFSA) - up to $7,500 pre-tax dollars can be set aside for dependent care eligible expenses like daycare, elder care, preschool, day camps. More details for DCFSA here.

FAQs

General OE Questions

I don’t want to make changes to my current benefits, do I have to participate in Open Enrollment and make elections?

  • No. This year is a passive enrollment, meaning if you are not wanting to change your current elections for the 2027 plan year, your current 2026 elections (excluding FSA/DCFSA) will be carried over to next year.

    • If you would like to make FSA/DCFSA contributions for 2027, they will need to be re-elected in Oracle (access via Okta).

  • As a reminder, Open Enrollment will run from October 26 through November 6th. Please make your benefits elections in Oracle (access via Okta).

What are qualifying events? If I experience one, what’s the process to add my dependent(s)?

  • A ‘qualifying life event’ (QLE) is defined by the IRS, and allows you to make changes to your benefits (i.e. add or drop dependents) outside of the open enrollment period. Common examples include marriage, divorce, the birth or adoption of a child, and loss of coverage for yourself or your spouse/domestic partner or dependents. Reach out to the Collective Health Benefit team by submitting an Employee Benefits Ping ticket for more information.

  • You have 30 days from the qualifying event date to make changes. You can do so by reaching out to the Collective Health Benefit team by submitting an Employee Benefits Ping ticket with information and documentation about your QLE. Your Oracle account will then be unlocked for you to submit changes.

Are domestic partners eligible for benefits coverage? How do we define a domestic partnership?

  • Yes. For the purposes of benefits enrollment, we define a Domestic Partner as someone with whom you share an interconnected domestic life (think, shared financial and domestic responsibilities). We do not require any legal certification of domestic partnership. Note that premium deductions for Domestic Partners are post-tax.

If I do not want to make changes and plan to elect the same plans for 2027, will I receive new insurance cards for 2027?

  • Medical: Yes! With the changes to the plan deductibles and out-of-pocket maximum amounts, all enrolled members will receive new ID cards for the 2027 plan year.

  • Dental/Vision: No. If you do not make a change to your Dental/Vision election for 2027, you will not receive new ID cards!

  • FSA  - Forma: If you currently have a Healthcare FSA/Dependent Care FSA, you will not receive a new card. If you did not have an FSA/DCFSA election in 2026, but will in 2027, you will receive a Forma FSA card in the mail in January.

  • HRA - Forma: No, HRA cards will not be sent out from Forma. To submit a reimbursement claim under the HRA please use the Forma website or app.

I can’t access my Oracle account. What should I do?

NOOM Questions

How much will accessing NOOM cost me?

  • Access to NOOM is free to all 18+ members enrolled in a Collective Health medical plan! Your NOOM membership is sponsored by Collective Health, so it’s free to you. NOOM Med is an optional clinical program that Collective Health has chosen for medically supervised weight management and behavior change guidance, plus access to weight loss medications (including GLP-1s), for those who clinically qualify. With NOOM Med, you can incur lab work and medication costs that will depend on your specific clinical case.

Can my dependents also access NOOM?

  • Yes! Any 18+ dependent enrolled in a Collective Health medical plan is eligible to participate in the NOOM programs.

I am already taking a weight-loss GLP-1. What will happen now that we have NOOM?

  • If you are currently taking a weight-loss GLP-1, you will need to transition your drug therapy to the NOOM program. After you enroll in NOOM, you will be able to indicate that you are currently taking a weight loss GLP-1. You will be required to provide your current prescription and evidence that you are actively following the prescribed therapy (showing that the most recent usage occurred within the last 6 weeks.)

Can I still get my weight-loss GLP-1 through RxBenefits/OptumRx?

  • No. If you would like to continue receiving a weight-loss GLP-1 through your Collective Health benefits plans, you will be required to go through the NOOM Med program.

What if I am taking a GLP-1 for Diabetes?

  • GLP-1s through NOOM will only be for weight loss! Any GLP-1 therapy specifically for Diabetes will continue as it does currently - through our Pharmacy benefit plan with RxBenefits/OptumRx.

Where can I review more FAQs about NOOM?

  • More FAQs about NOOM can be found here.

CH on CH Questions

I have feedback about the platform - how do I reach out?

  • Any feedback about the Collective Health platform can be submitted directly to the Benefits team via Employee Benefits Ping ticket (be sure to select “CH on CH” as the ticket inquiry type). The team will then pass your feedback along to the appropriate team(s). It is important that this process be followed in order to ensure your feedback is directed to the appropriate team. Please do not reach out directly to your colleagues via slack or email.

Will Care Navigation be available on 01/01/2027?

  • No, Care Navigation will not be available on 01/01/2027; however, we are looking into adding it in the future. However, we do have Care Suggestions on the platform.

Healthcare Questions

I am an Utah employee - what is the difference between the Blue Shield Preferred Blue Option Network, and the Blue Shield BlueCard Network?

  • The Blue Shield Preferred Blue Option Network offers most of the Intermountain Health network as INN (UofU Health is OON).

  • The Blue Shield BlueCard Network offers the University of Utah Health network as INN (Intermountain Health is OON).

I’ve noticed we have different premiums for hourly vs salaried employees. Why is that?

  • Our benefits cost sharing strategy aligns with the company’s business strategy to provide no barriers to access healthcare. In order to allocate costs to help achieve that goal, we have different premiums for employees in hourly roles vs those in salaried roles. Collective Health continues to bear the majority of the cost, making healthcare affordable and accessible to all employees.

How can I access virtual care on our Blue Shield plans?

  • Blue Shield offers a telemedicine option called Teladoc. Teladoc provides you 24/7 access to virtual doctor visits via their app with a $0 visit copay. To access Teladoc, login to your MyCollective account, navigate to Get Care -> Additional Benefits -> Teladoc.

HRA and FSA Questions

What is an HRA and how is it different from an FSA?

  • Health Reimbursement Arrangement (HRA) is a tax-advantaged account funded by Collective Health to help cover your health care costs. The money contributed to an HRA is not taxed. You can access these funds to pay for any qualified medical expense for you or your dependents.

  • Only your employer can contribute to HRA; whereas a Flexible Spending Account (FSA) is funded by employee contributions.

What are the FSA maximum elections, and if I enroll in an FSA product, will the $ amount expire at the end of December 2027?

  • The IRS announces the annual maximum contribution amounts each year and have not yet announced the 2027 limits. The 2026 maximum amounts are:

    • Dependent Care FSA - $7,500

    • Healthcare FSA - $3,400

  • If you enroll in a 2027 Healthcare FSA, you’ll want to elect the amount you anticipate needing for the next year, as the funds will expire after December 31, 2027 (with up to $680 {pending IRS confirmation} of unused funds eligible to carryover to your 2028 Healthcare FSA ). Otherwise, you might need to spend some time on FSAstore.com!

  • If you enroll in a 2027 Dependent Care FSA, the balance will expire after March 15th, 2028, with no carryover allowed. You have until March 31st, 2028 to submit eligible claims incurred in the 2027 plan year.

  • FSA plans must be elected each year, your elections will not carry over.

How do I enroll in Commuter Benefits?

  • Everyone will be provided access to the Forma portal. If you’d like to enroll in a commuter plan(s), please enroll and elect your monthly contribution amount directly in Forma’s portal. You can change your election amount on a monthly basis. The deadline to submit your monthly election is the 15th of the prior month. For January 2027, please submit your commuter plan(s) contribution amount by December 15th, 2026.
